Server crashes when people leave

I got a server running fine and everyone, but when someone leaves it has the chance of crashing. At first I thought it was when my brother left because he's on the same router as me. But then my friend left and it crashed...

I have sourcemod, metamod and SM Super Commands installed. Apart from that it's default Obsidian.

do you use sv_cheats 1 at any point?

also you might want to take a look at the sourcemod forums there is a disscussion about that (just search for rcon exploit)

No sv_cheats wasn't at 1 at any point.

I don't think it's an rcon exploit, because it happened with people I know would not do that

It'll be a while until I can test it again because, when I leave it doesn't seem to crash
